Transition

At age 14, special education students are entitled to have, as part of their IEPs, a “transition plan” to adult life.  The transition plan provides services to older students while they are in school to help them achieve their objectives in a variety of post-school settings, including not only employment and vocational training, but also post-secondary academic education for those who are able and family and community living skills for those who are more severely disabled. Transition services must be designed within an outcome-oriented process based on the individual student’s needs, taking into account that student’s preferences and interests.

We can help you identify and obtain a transition plan that meets your student’s needs for the future.  We can help you prepare for your IEP conference, or attend it with you.
